We are currently seeking a Documentation Reviewer I to join our team! The ideal candidate for this role has excellent communication and interpersonal skills, proven analytical and problem-solving ability, experience with Microsoft Office, the ability to learn and use project management tools, can maintain attention to detail under varying scenarios, and is skilled at time management and meeting deadlines.
A day in the life!
You will be responsible for the following:
- Reviewing and providing guidance to clinical staff of clinical documentation for appropriateness, congruency, and accuracy as it relates to Coding, OASIS, and 485 plan of care development according to CMS guidelines, coding guidelines, and OASIS conventions
- Conducting quality assurance documentation reviews of assigned records to include but not limited to OASIS for documentation inconsistencies and accuracy
- Reviewing plan of treatment for accurate reflection of patient's condition and skilled interventions
- Reviewing medication profile for relevance to interventions, goals, and coding sequencing
- Care planning as it relates to assessment and regulatory requirements
- Applying clinical expertise to patient documentation and Plan of Care to identify missing or inappropriate patient interventions
- Providing customer service/education and acting as a resource to agency with regards to CMS guidelines, Home Care Coding, PDGM guidelines, and billing-related issues
- Reviewing and coding coding OASIS documents and plan of care with productivity maintained at the weekly target set by the management team
- Communicating with service offices with a focus on documentation trends, star ratings and potential process issues
- Conducting reviews of coding and providing recommendations for appropriate coding based on current coding guidelines
- Maintaining proficiency and knowledge of OASIS Data set items, home care reimbursement, and compliance as related to OASIS regulations and requirements
Do you have what it takes?
Required Qualifications:
- 3-5 years HH and Hospice experience
- Must be an RN
- One year experience in Coding and OASIS Reviews.
- Intimate knowledge of multipleelectronic health records.
- Knowledgeable in the Conditions of Participation (CoP's)
